Butler Chain of Lakes Safety Observations Submission Form
Use this form to provide information on perceived safety issues and potential violations of regulations controlling water recreation and watercraft operation. These observations will be collected and used to inform interested parties about the current safety climate on the Butler Chain of Lakes. All submissions will become public record, available to all. Please provide as much information as possible, omitting any details you cannot or do not wish to include. If you can include photographic or video observations, please follow the instructions below on how to upload those files to the online repository.
